WHAT IS BLEPHAROPLASTY?

Blepharoplasty, commonly known as eyelid surgery, is a highly effective procedure for rejuvenating the eye area. By addressing excess skin, fat prolapse, and laxity of the eyelids, both upper and lower blepharoplasty can restore a brighter, more alert appearance. In Jonathan’s practice at Montrose London, blepharoplasty is supported by advanced recovery protocols and complementary treatments to optimise healing and results.

WHY CONSIDER BLEPHAROPLASTY?

Blepharoplasty, or eyelid surgery, offers significant benefits that go beyond cosmetic enhancement. By carefully removing excess skin and fat, this procedure rejuvenates the eye area, reduces signs of aging, and provides a more youthful, rested appearance.

  • Upper Eyelid Rejuvenation: Effectively addresses loose skin and drooping eyelids, which not only refreshes your look but can also improve peripheral vision.

  • Lower Eyelid Correction: Targets eye bags and puffiness to smooth the under-eye area and restore a revitalized look.

  • Patient Comfort: Typically performed under local anaesthetic, ensuring the surgical experience is pain-free for most patients.

  • Enhanced Quality of Life: Beyond the aesthetic improvements, patients often experience a significant boost in self-confidence and long-lasting results.

  • Functional Improvements: By lifting and tightening the lids, the procedure creates a more alert appearance and can resolve issues like eyelid heaviness or numbness.

  • UPPER BLEPHAROPLASTY

Upper blepharoplasty surgery focuses specifically on the upper eyelid area, where the primary goal is to remove excess skin and occasionally fat. This procedure is ideal for creating a smoother, more youthful contour while effectively alleviating issues such as hooding or a heavy, tired appearance. By addressing these concerns, the surgery helps to open up the eye area and can even improve the field of vision for those experiencing significant sagging.

  • LOWER BLEPHAROPLASTY

Lower eyelid surgery, or lower blepharoplasty, is designed to treat the area beneath the eye by removing or repositioning excess fat and occasionally removing skin. This approach is highly effective at reducing the appearance of eye bags and puffiness that often develop with age. By refining this area, the procedure softens the transition between the eyelid and the cheek, resulting in a more refreshed and revitalised look.

  • COMBINED AND SPECIALISED TECHNIQUES

For those seeking a total transformation, a combination of upper and lower blepharoplasty may be recommended to achieve comprehensive rejuvenation of the entire eye area. Modern surgery offers various techniques ranging from traditional incisional methods to less invasive  ‘scarless' approaches, allowing a consultant plastic surgeon to tailor the plan to your specific anatomy. This personalised strategy ensures that whether you are addressing excess skin, fat, or both, the final results remain natural and balanced. A brow lift is often combined with upper blepharoplasty for the best result.

WHAT TO EXPECT

During the early recovery period, it is normal to experience swelling, bruising, and temporary tightness around the eyes. Some patients may also notice dry eyes, numbness, or surface irritation following surgery; prescribed eye drops are often recommended to manage these symptoms. While vision may feel slightly blurred initially due to ointment or swelling, the eye area will continue to refine as healing progresses, with final results appearing over several months.

To ensure the best possible outcome and a smooth recovery, please note the following:

Pre-Surgery Preparation: Please avoid using laser treatments or retinols for at least two weeks prior to your procedure.

The Clinic Experience: We maintain a relaxed and reassuring atmosphere. You will undergo final checks before the procedure begins; if you are having the surgery under local anaesthetic, you are welcome to eat and drink normally beforehand.

PATIENTS RESULTS

WHAT RESULTS ARE TYPICALLY ACHIEVED WITH EXCESS EYELID SKIN REMOVAL

Patients typically experience brighter, more open eyes with reduced heaviness, puffiness, and under-eye shadows. Upper blepharoplasty, often called an eyelid lift, is one of the most common facial cosmetic surgeries as of 2026. Many patients seek this cosmetic surgery to address a hooded appearance caused by development of excess skin and a lower brow position, which can affect both appearance and vision. As people age, the brow position falls and skin stretches, leading to heavy skin on the eyelids that creates an overhang and contributes to a hooded appearance. Improving the eyelid area can enhance self-esteem and psychological well-being. The eyes appear more rested and youthful, while results remain subtle and natural. Outcomes are long-lasting, although natural ageing of the skin continues over time.

  • 1 WEEK POST OP

You will notice the initial stages of healing where swelling and bruising are most apparent, though the new contour of the eyelids is already beginning to emerge.

  • 6 WEEK POST OP

The majority of the inflammation has subsided, revealing a much smoother, more rejuvenated appearance as the tissues continue to settle and refine.

Blepharoplasty is performed by Jonathan Dunne, supported by experienced nursing and anaesthetic teams. The procedure is usually carried out under local anaesthetic with or without sedation, but general anaesthetic may be used for longer or more complex cases to ensure patient comfort and safety. Care is delivered with attention given to surgical precision and post-operative recovery planning. Meticulous care at the surgical site is essential to minimise complications such as infection or delayed healing and to support optimal recovery. Patients are usually able to go home on the same day after blepharoplasty surgery.

  • IS BLEPHAROPLASTY PAINFUL?

Discomfort is usually mild and well managed with prescribed medication. Most patients report that the procedure itself is pain free due to the use of local anaesthetic or sedation. Post-operative discomfort typically involves mild swelling and tenderness around the eyelids, which can be alleviated with over-the-counter painkillers or medications prescribed by your surgeon. The healing process is gradual, and any discomfort usually subsides within a few days to a week.

HOW LONG WILL SWELLING AND BRUISING LAST?

Bruising and swelling typically improve significantly within 7–10 days, with subtle swelling resolving over several weeks. It is common for swelling to peak around 48 hours post-surgery and then gradually diminish. Skin discoloration may also occur after upper blepharoplasty, often associated with bruising or swelling, and is usually temporary. Patients are advised to use cold compresses during the initial recovery phase to reduce swelling, but the eye area should be kept dry for a few days. Keeping the head elevated while resting is also recommended. Complete resolution of all swelling and bruising can take up to several months, but most individuals can return to social activities within two weeks.

    WILL SCARS BE VISIBLE?

Incisions are carefully placed within the natural fold of the eyelid or inside the lower eyelid, making scars discreet once healed. After the procedure, the wound edges are meticulously closed with fine sutures to ensure minimal visible scarring, and fine strip dressings are applied to protect the wound. The strategic placement ensures that scars blend with the natural folds and lines of the eyelids, becoming nearly invisible over time. Proper post-operative care, including keeping the incision sites clean and avoiding excessive sun exposure, can further minimize scar visibility. In some cases, your surgeon may recommend specific creams or treatments to help optimize scar healing.

  • WILL INSURANCE PAY FOR HOODED EYELID SURGERY?

Insurance coverage for hooded eyelid surgery, also known as upper blepharoplasty, depends largely on the specific circumstances and the insurance provider's policies. Generally, if the surgery is performed for functional reasons—such as improving vision obstructed by saggy upper eyelid skin—insurance companies may cover part or all of the procedure. To qualify, patients often need to provide documentation from an ophthalmologist or oculoplastic surgeon confirming that the excess eyelid skin significantly impairs peripheral vision or causes other medical issues. Cosmetic blepharoplasty, performed solely for appearance enhancement without functional impairment, is typically not covered by insurance.

CAN INJECTABLE TREATMENTS ACHIEVE SIMILAR RESULTS TO BLEPHAROPLASTY?

Injectable treatments, such as dermal fillers and Botox, can improve certain aspects of the eye area’s appearance, like smoothing fine lines, reducing wrinkles, or restoring volume loss. However, these non-surgical options cannot remove excess skin, remove excess tissue, or reposition excess fat—issues that are specifically addressed by surgical procedures like upper blepharoplasty. Injectables also cannot correct significant eyelid hooding. Therefore, while injectable treatments may complement blepharoplasty or serve as temporary measures for mild concerns, they do not provide the comprehensive and long-lasting results achievable through eyelid surgery blepharoplasty. Patients seeking to address saggy upper eyelid skin, pronounced puffiness, or excess tissue usually require surgical intervention for optimal outcomes.

HOW LONG DO THE RESULTS FROM A BLEPHAROPLASTY LAST?

The results of upper blepharoplasty are generally long-lasting, often enduring for 7 to 15 years, depending on individual factors such as skin quality, ageing process, and lifestyle. While the procedure effectively removes excess skin and fat, it does not stop the natural ageing process, and over time, some skin laxity or changes may recur. Many patients enjoy a more youthful appearance for years before considering further treatment.

However, if the initial outcome is unsatisfactory or if results change over time, further surgery or revision surgery may be required to correct issues such as contour irregularities or eyelid function problems. It is important to note that additional costs may arise if revision surgery is needed within 12 months of the initial procedure. In some cases, additional procedures, such as surgical revision or adjunctive cosmetic treatments, may be recommended to maintain or enhance results as part of ongoing facial rejuvenation.
